FULL PAYMENT IS DUE WHEN SERVICES ARE RENDERED.
We accept Cash, MasterCard, Visa and Discover.
Appointment Policy
A 24 hour notice for cancelled or rescheduled appointments (Saturday & Sunday excluded) is required or an Office Visit fee will be charged. Monday appointments need to be cancelled by Friday. This allows us time to schedule another patient and the time is not lost.
Insurance
We are not accepting health insurance at this time. If you would like to file a claim with your insurance company, be sure to inform the front desk administrator upon arrival and we will ensure proper codes appear on your detailed receipt. You may then send to your health insurance company the detailed receipt along with a claim form provided by your insurance company. This office does not submit any claims or accept any insurance payments. Medicare and Tricare do not offer coverage for acupuncture.
Initial Visit
A New Patient Detailed Health Evaluation with the physician is conducted on your initial visit. The physician will determine the type of treatment that is needed after the evaluation has been conducted. Not all patients receive acupuncture, therefore, the cost of the health evaluation does not include treatment. Acupuncture treatments may be administered immediately following your evaluation.
